IMPORTANT! Please read before proceeding. FORMS

The following contains all the project research forms that are required for the Pittsburgh Regional Science and Engineering Fair (PRSEF). Teachers may download these pages and distribute them to the students for completion. The School Registration Form and Fee must be submitted by November 16, 2009.

PRSEF is an affiliate fair of the International Science and Engineering Fair (ISEF) and must comply with Federal regulations ensuring the safety and well-being of humans and animals involved in student research, as well as the safety and welfare of the student researchers themselves. School Registration Form
Homeschool Registration Form  Note: Homeschool is defined by the address of the Homeschool. The registration fee covers any student under 21 years of age that resides at that address. If additional students not living at this address attend, those students must complete a Homeschool School Registration Form and fee.

c) Abstract Form

My project involves people and I did experiments on them
a) Form 1, 1A includes Research Plan, 1B, 4 AND 1C if conducted in a Regulated Research Institution. Form 2 is required if more than minimal risk is involved.       (Download forms from ISEF website)

My project involves potentially Hazardous Biological Agents
(previously classified as pathogenic and potentially pathogenic agents, recombinant DNA, and human and vertebrate animal tissues)

(includes prescription drugs, consumable ethyl alcohol and tobacco)
